Garfield County Memorial Hospital & Long Term Care proposes to develop a resident directed model of care designed to ensure that the elderly who reside in their community retain the self-sufficiency, autonomy and dignity they have known throughout their lives.  We believe that through the implementation of this program, we will enhance the quality of life for residents, improve the quality of work environment for staff, increase operating efficiencies as well as promote greater resident autonomy.

We believe that our project will serve as the benchmark for the thousands of small rural providers throughout the nation who have the interest in advancing resident centered care, but perceive that they lack the resources to do so.  It is our hypothesis that with a relatively small capital budget, and with the engagement of staff, trustees and community, that we can develop and sustain a resident-centered care model that exemplifies the very best of rural living: respect, self-sufficiency, autonomy and neighbors supporting neighbors.
